Leaving hospital

If you or a member of your family are concerned about how you might manage your home when you leave hospital then we may be able to help you.

We have officers that work with patients, their families and colleagues in the NHS to make sure all patients are able to go home safely and in a timely manner. Some of the things this might include are:

  • Working with an occupational therapist to make sure any adaptations are in place
  • Helping organise moves to alternative accommodation
  • Providing small items such as specialised bedding, key safes and food parcels etc.
  • Providing deep clean services.

I have no fixed address

If you have no fixed address when you are admitted to hospital please let the hospital staff know as they will automatically ask us to visit you on your ward so we can help you with your housing situation.
