Where do I get an application form?
Before you start receiving the service, your support provider should provide you with an application form for Supporting People Grant (.pdf, 223KB).
How do I complete the form?
Your service provider should help you to complete the application form. This is normally done at the same time as completing your application form for Housing Benefit. It is really important that you complete section 3 and sign the form because it is highly unlikely that Supporting People grant will be paid without doing so.
Where do I send the completed form to?
Your service provider should post the completed application form to the address printed at the end of the form.
