The Adult Placement Team is part of the Council's Department of Adult Services. It aims to provide people with the opportunity to form new and meaningful relationships and to broaden their experiences. Care is provided by people approved by the team (called 'approved carers'), in their own home and, in some circumstances, the home of the person who needs the care.
The service is offered to adults over the age of 18 who may benefit from such a service tailored to individual needs, that is:
- older people
- people with a learning disability
- people with a physical disability
- people who may be experiencing mental health problems.
The service is flexible and tailored to meet individual needs. People who use the service have the opportunity to spend time in a 'homely' environment, where they are welcomed as 'one of the family' and are able to experience the warmth and care that is usual in the setting of an 'ordinary' home.
Support provided includes:
- Short term breaks - Specified periods of time spent in the approved carer's home, which involves overnight stays when necessary.
- Sessional care - A service offered on a regular basis, which does not involve an overnight stay.
- Long term care - A service whereby the person receiving the care lives with the approved carer on a long term basis as 'one of the family'.

Once you make a referral, we will do an assessment of your needs to decide whether or not you are eligible to receive services. We use Department of Health guidance 'Fair Access to Care Services' to work out your eligibility.
