What Happens Next?
The social worker will ask about your health, ability to carry out daily tasks and about support from friends and/or family. They will help you to think about social and educational activities as well as volunteering and employment opportunities. In some cases, with permission, they will contact other professionals that work with you, such as doctors, nurses, physiotherapists and occupational therapists to learn more about your needs.
If you have an unpaid carer helping to look after you, the social worker will also talk to them to offer them an assessment of their needs, as a carer.
We use the Government’s 'Fair Access to Care Services' guidelines to help us to decide whether or not you are entitled to care services. We may be able to offer you advice and some other help, or put you in touch with other organisations who may be able to help.
Paying for community care services
We charge for care services. How much we charge depends on a financial assessment which will be completed by the Financial Assessment Team. As a result of that assessment, you may receive a free, reduced or full-cost service.
