Trained staff in all centres aim to deliver services in a 'person-centred' way, designed to promote the health and well-being of people in their care (service users). Staff do this by devising an individual support plan, using information obtained from the service user, their carer(s) and from their social work assessment.
A support plan could consist of some of the following elements:
- Emotional support
- Mental stimulation
- Physical stimulation
- Social interaction
- Special dietary requirements
- Transport arrangements
- Personal Care support
