Referrals

We accept referrals from social workers but not directly from individual members of the public. If you think that Fourways may be appropriate for your needs, please contact your social worker.

If you do not have a social worker and live in Wigan, contact the central duty team and ask for a social work assessment.

This assessment helps the social worker to decide whether or not you are eligible to receive services. We use the Department of Health guideline 'Fair Access to Care Services' to work out your eligibility.
