Summary of Model Forms
Visit Checklist (Form 1). Can be used by the School Educational Visits Co-ordinator/Group Leader at the outset of the planning process. To be retained at school.
Category B Notification Form. To be completed by Group Leaders for category B type of off-site activities. A copy of this to be retained by the Educational Visits Co-ordinator. A copy of this to be sent to the EVA at least 10 days prior to the date of the visit.
Category C Notification Form (Form 2). To be completed by Group Leaders for category C type of off-site activities. A copy of this to be retained by the Educational Visits Co-ordinator. A copy of this together with a full Risk Assessment to be sent to the EVA at least 4 weeks prior to the date of the visit.
Form 3 Evaluation of the visit or off-site activity. Can be used by all Group Leaders immediately following the visit. A copy of this to be retained at school by the School Educational Visits Co-ordinator. For activities falling within Category B or C, a copy to be sent to the EVA.
Form 4 Incident/Accident form. To be completed as soon after the incident as possible to recall information accurately for any potential follow up.
Form C1 Parental Consent. Form C1 for regular Category A & B off-site visits/activities. To be retained at school.
Form C2 Parental Consent. Form C2 for Category C visits. To be retained at school.
Form RA1 Risk Assessment Questionnaire This form to be completed by the Group Leader, gathering as much information as possible on the venue to be visited. This form must be signed by a representative from the establishment being visited. A copy of this is to be retained by the school.
Form RA2 Assessment of Risk To be completed for all off-site visits. A copy of this to be retained by the school and a copy sent to the EVA for activities falling within categories C.
Form EVC1 Educational Visits Register For recording all visits. To be maintained by the Educational Visits Co-ordinator and retained at school.
Form EVC2 Staff Experience & Qualifications Register To record details of each individual member of staff (including regular volunteers) who are involved in visits, to catalogue their roles/responsibilities and progressive development. This will help the Educational Visits Co-ordinator assess the suitability of staff for different activities/visits. To be retained at the school.
Form EVC3 Group list To list of all involved in a visit, to be completed by the Group Leader for every off site visit.
Form EVC4 Swimming Pool Checklist To be completed for visits to swimming pools.
Form D1 Volunteer Drivers Form To be completed by all parents willing to transport on school visits. To be retained at the school.
