NQT Assessment Forms

This page gives information on how to submit assessment forms and how to request an extension.

Please ensure to return all forms within two weeks from the beginning of the following term., Where forms are completed part way through term, they should be sent to the address below within 10 working days of completing the assessment form. The forms must include,

  • all requested information including number of whole days worked during the assessment period
  • signatures of the NQT, induction tutor and head teacher and;
  • Strengths, areas for development, targets and support provided to the NQT.

Complete when a NQT has joined a new school or approved learning establishment, please download the document below and ensure you complete all sections.

Interim, First or Second Assessment Forms

Complete when a NQT has worked a full calendar term on a full time contact or equivalent to a full time term on a part time contract. This form should not be used for the final assessment form.  

Interim Assessments: Are to be completed when NQTs have not completed the term and will be moving to another school. This form should be completed at the end of that term to provide information about NQT progress. The NQT should take this form onto their next school.

Final Assessment and Recommendation Form

To be used where a NQT has successfully completed a third assessment period and the total number of days for their induction.

Applying for a 12-month extension to supply

From the first day of short-term supply, NQTs may continue for 16 consecutive calendar months. This is regardless of the working hours of the NQT or the duration of that short-term supply work. For example, if an NQT starts short-term supply on 1st January 2012, their entitlement to short-term supply ends of 30th April 2013, regardless of how many days they have worked, or whether they did inductable work in between.

Once this 16 Months has passed, NQTs working in maintained schools or non-maintained special schools should cease short-term supply work unless they are granted an extension to the 16-month supply rule. Any further short-term supply undertaken without an extension is illegal unless undertaken concurrently with inductable work.

The NQT Induction Team is required at their discretion to grant an extension of up to a further 12 consecutive calendar months (no longer period is allowed, nor is any break in this period allowed). Any previous work undertaken illegally must be deducted from this period. The application form and the email address to apply for an extension are shown below.

Please note:

  • The granting or withholding and length of any extension is at the discretion of the Induction Team and any extension should not be regarded as an entitlement.
  • Extensions may only be granted where "exceptional circumstances" are seen to apply. You will be expected to provide information to explain your circumstances and you will be expected to provide information about the posts you have applied for.
  • Extensions granted by Wigan Local Authority apply only to those schools within their area of responsibility. If you wish to work separately in other local authority, areas please contact the appropriate body in that area for an extension.
  • The Induction Team is not empowered to extend beyond 12 months and may only grant one extension. Once an extension has begun, it cannot be suspended or re-dated if you then gain inductable work during that period.
  • As far as possible, the extension will start from either the date requested or the first day of short-term supply work carried out past the initial 16 months. If you have carried out any supply work illegally, the extension must be backdated.

If you wish to apply for an extension, please write to Sharon Adams, Learning and Development, The Learning Centre, Park Road, Hindley, WN2 3RY or email Learninganddevelopment@wigan.gov.uk with your request, approximately one month before you require your extension to begin.

When contacting Wigan please include the below information in your initial application.

  • On which date did you achieve Qualified Teacher Status (QTS)?
  • On which date did you complete your first full day of short term supply?
  • Have you completed the full 16 calendar months within Wigan Borough? If not in which authority?
  • Please provide information on the inductable positions you have applied for / completed and what supply positions you have completed in the 16 calendar months.
  • Do you have time remaining within the 16 calendar months? If applicable how much time is remaining? If you have worked longer than the 16 months please provide the exact dates between your first full day of supply and your most recent full day of supply.
  • Have you applied for an extension with another authority? If the application has been granted please attach a copy of the confirmation letter, if the application has been rejected please attach a copy of the rejection letter.
