FUNd4ME Extended Services Funding Proforma

This proforma will be used primarily by the ESDW to organise cluster activities using the various funding pots available e.g. Start Up Funding, FUNd4ME, Income generation.

This needs to be completed for every child who qualifies for the FUNd4ME and wants to access any Varied Menu of activities either organised by the school, the cluster or takes place in the local community. This will link in with the financial monitoring documents to track payments through invoices, receipts etc.

Parent Disclaimer Form

This needs to be given out to parents of pupils who qualify for the FUNd4ME and are taking parting activities NOT organised by the school or the cluster. This means the FUNd4ME can pay for the activity but that the necessary checks haven’t been made and it is the responsibility of the parents / carers to do these checks. This will be used in conjunction with the NSPCC’s ‘Out Alone’ document.
